Parental Permission Medical Release Form - This form MUST be printed, filled out, signed and brought to camp by every girl attending.

Photo Release Form - This form is not needed if the photo release questions are answered in the Online Registration to help us know if we can share photos and/or videos on the Fargo YW Camp Facebook Page, website or slide show.
